Patient Management Programs
We can use
health
screens and/or the
pre-certification process as a means of red-flagging
cases for follow-up care. Our goal with case
management is different from what is traditionally
seen. Instead of simply directing patients towards
the cheapest services, we guide them through their
healthcare experience. We lead them through
appropriate utilization of services and improved
efficiency of care, ultimately
leading to reduced costs through greater value.
We break our patient
management programs down into 3 categories as a
means to aid in the understanding of the management
components and as a way to help focus a program
objective. Depending on client needs, these
components can be combined into a single program.
